Mr Avi Agrawal was awarded the iWantGreatCare certificate of excellence in 2025 for delivering outstanding care.
As a fully trained Oncoplastic Breast Surgeon, I carry out regular diagnostic fast track breast clinics and a full range of breast surgery, including sentinel node biopsy with intra-operative analysis, oncoplastic surgery and breast reconstruction. I also have a special interest in and undertake surgery for skin cancer. I graduated from the London Hospital Medical College, University of London. My general surgical training was based around several centres of excellence in managing breast diseases, including The Royal Marsden Hospital where I developed oncoplastic techniques to optimise the cosmetic outcome of breast surgery. I have always been interested in cancer surgery, and as part of my higher surgical training at the University College London, I was awarded a master's degree for my research into Gene Therapy for the prevention and treatment of cancer. In 2007 I was appointed Consultant Surgeon at Portsmouth Hospitals NHS Trust. I have been the Clinical Lead/Head of department for the Breast and Endocrine Surgery and Surgical Oncology department for Portsmouth for over 10 years. I have also chaired and lead the Breast Multidisciplinary Team at Queen Alexandra Hospital for over 7 years. I am the Surgical Lead for breast screening cancers and also the Joint Regional Lead for breast cancer for Wessex. I am the Lead Trainer for the Portsmouth National Oncoplastic Fellowship Program and have helped many trainees develop their skills for their consultant career.
Outstanding care from Mr Agrawal, he listened to my issues and explained everything in detail, my experience was perfect and I couldn’t fault anything. Would highly recommend having treatment with Mr Agrawal. Could not be happier with my results.

Before I had my surgery I spent a long time researching surgeons. Mr Agrawal exceeded all my hopes and expectations. From my initial consultation to my surgery (Bi Lateral reduction) he was incredibly clear regarding all process, fees, and and result expectations I may have. I was quite fixed on the result I would like and Mr Agrawal was very clear that results vary based on a whole host of different reasons. To the point that he asked me to go home and think about it thoroughly to ensure I was absolutely sure that I was happy to proceed on that basis. I was, and a few months later I have now had my surgery. Mr Agrawal, all nursing and care staff and the anaesthetist were all fantastic. I felt completely comfortable and safe. I cannot recommend Mr Agrawal and his team enough.

From the very first consultation at the Queen Alexandra Hospital through to surgery and post-operative care at Spire, Portsmouth, I was treated like an individual whose thoughts and feelings were taken into account. Mr Agrawal is gently persuasive and clearly very experienced and respected, and I've appreciated being kept informed on every step of the breast cancer journey.

I had an excellent experience with Mr Agrawal. He was very clear in explaining the facts, set realistic expectations, and always took the time to reassure me. His calm, confident manner made me feel I was in the best hands, and I truly believe he had my best interests at heart. The surgery went smoothly, and I am very grateful for his skill and care.

Mr Agrawal took a lot of time and trouble to explain all my options for surgery and was happy to reepeat after I had not taken the information in. He took the trouble to see me several times on the day of the surgery to make sure I was ok or if i had any questions. His skill in surgery resulted in an amazing outcome for me. I am extremely grateful.

Mr Agrawal my breast cancer surgeon was wonderful. He was knowledgeable, a real expert but in a way that ensured you understood what was happening. He was so reassuring through a very difficult time. He also completed a very successful surgery, not only removing all the cancer but doing the reconstruction that delivered a good outcome.

My unsightly and uncomfortable big lump created by a ruptured implant has been skilfully dealt with by Mr Agrawal. I envisioned painful days after the operation, but my recovery proceeded with hardly any restrictions and no pain.

I recently had breast augmentation surgery at the Spire Hospital in Portsmouth carried out Mr Agrawal. He is a master of his craft and I am over the moon with my results. From the initial consultation to my surgery everything was seamless. Any questions I had were answered, I was sent away with information after my initial consultation to read through at my leisure. I felt I was making an informed decision when deciding to go ahead with my surgery and felt at no point was under pressure to proceed if I didn’t wish to. My recovery has been amazing, very mild discomfort as to be expected after surgery. My scars are minimal and extremely neat. I would recommend Mr Agrawal in a heartbeat for anyone considering this type of surgery, his warm nature makes you feel at ease and the results speak for themselves.

I'm very grateful to Mr Agrawal. Dealing with difficult news is never welcome, but he walked me through my situation, set my expectations and discussed my options for treatment compassionately and professionally. He was always happy for questions, whether they were about the science, the procedures, possible consequences and even brought a welcome thread of humour into the conversation too! What really made him stand out though, was the fact that he seemed to take ownership of my case and helped work the system to bring about prompt consultations, scans and treatment after some unexplained delays waiting for a second biopsy. This included guidance on the best way to navigate both NHS and private provision and finding ways around situations where other clinicians were fully subscribed or unavailable. Navigating the healthcare system can feel impersonal and industrialised. It can also be haphazard too with so many poor processes before the pressures on capacity start to tell. It's really motivating to see someone taking responsibility for the performance of their team, to say nothing of the time-related clinical benefits prompt intervention can bring. Mr Agrawal clearly has a passion for his work and the welfare of his patients and I was delighted to be treated by him.
