Following Brachytherapy in March and 21 doses of Radiotherapy in April 2020 for Prostate Cancer I thought I had come through with minimal side effects. However in July I started to develop what I concluded was Radiation Proctitis. Getting treatment was very difficult due to Covid restrictions and it was a very slow path of investigation until an endoscopy in late October which, after a two week wait for the results, gave final confirmation of the diagnosis. But, by early November I had developed severe retention requiring hospitalisation and catherisation and there was a lag of 3 weeks with no treatment for the Proctitis at which point the difficulty of getting Urology and Colorectal to talk to each other became apparent, despite the proximity of the two problems. Mid November and a Colorectal hot clinic led to a second hospitalisation again for retention (urology) issues but my luck changed as I was referred on the Monday for Sigmoidoscopy treatment for Radiation Proctitis with Miss Kathryn McCarthy. She immediately offered a specialised treatment under which she applied Endoclot to the damaged area which by this time was continuously leaking blood. I was able to watch the whole procedure and she carefully explained that the powder that she used to dust the damaged area was to aid clotting. A further appointment was made for three weeks later and again for mid January. Her enthusiasm for the improvement in the damaged area was infective. the damaged area has shrunk to about a third of its previous size, with the upper level almost clear, the proctitis very much improved and further up there were no signs at all. She proceeded to remove a polyp from my colon and said that this would be analysed within 2-3 weeks but did not expect any issues and the sprayed Endoclot over the much smaller area still needing treatment. Argon Plasma Coagulation treatment was then applied to the lower level of Proctitis to cauterise the area that was too low to retain and respond to the Endoclot. Three treatments in one! I was then advised that I would need to be seen again in 4-5 weeks. I count myself as very fortunate to have been referred to Miss McCarthy who I believe is one of the few surgeons qualified to apply Endoclot at Southmead. I have found her cheerful, informative and always willing to commentate on what she is doing as I watch on screen. I have been advised that Radiation Proctitis is difficult to cure but I am very pleased with progress to date and hopeful of solution.
