everything she said she would provide, she provided. very friendly, in difficult circumstances. very helpful. i was in a cell for long time so nice to talk to somebody like I was a human being & worth something.

survey taken from patient. Patient considers herself very lucky she is 91. [name removed] has never smoked or does not like drinkiing. Her favorite drink is tea. [name removed] has had more trouble with the renal department than the triage - she has also switched to non paperwork with renal clinic. She tries if she can she has a fortnight before kidney blood test happens she tries to link it with inr but now she is paperless she does not get form to place with actual sample. Last time she contacted renal team 4 times and she asked for them to send her an email but they would not do this. Why?

survey taken from pts wife. She thinks DNs are excellent and she has never been involved before with anything like this. Pt was diagnosed with Parkinsons and dementia earlier this year. Right from the start all the HCPs from all the services have been excellent.
