Poor communication with family. 4 days in A&E waiting for a bed because bed on ward broken promises of help but always delayed. On ward good care but was not aware/informed when medication increased although might be my confused state. {} for planning review and discharge meetings- distressing.

Suggested improvements
Family left uniformed {} nobody available to answer wad phone. Ward clerk but seems to have no responsibility for answering phone or keeping next of kin informed. Shortage of staff was apparent took a week for GP to receive discharge letter community mental health plan couldn't been addered to.
