The community team following contact from family 5 days later were all amazing and very caring.
Suggested improvements
Unfortunately the referral from the hospital and the community team was very poor. We were promised so much (which we eventually got) but need on pay I at have not day 5, had I not had knowledge and {} number (though my work) we would have been left with little help.
