Problems persist with poor communications.
1) Between doctors and patients not being fully informed about their tratment plan.
2) Between the different local health care trusts with slow to non existant access to patient records when a patient has been treated across trusts.
3) Between hospital and patients GP.
All of which do cause much stress and confusion for the patient having to fill in and chase 'the system' where these frequent gaps occur.
