Care ok, but discharge procedure failed this time. Old lady who was not encouraged to get on her feet or use her legs for 3 weeks while in your care. Allowed home & carers set up but no equipment put in place for the lady for her return. Family totally unhappy with the care on discharge & home return.

Have relevant equipment in place, instead of passing the responsibility to someone else
