I have given these answers because it took a long time to get my first appointment with the team and since then I've had minimal and inconsistent communication. The written plan from my psychiatrist to my GP had mistakes and mentioned things I don't remember discussing during my appointment. I have called several times for clarification and had no response. I am very concerned that I will be discharged from the team without a plan of care in place of sufficient diagnosis to recieve appropriate care.

Suggested improvements
Better communication. When I call us like to speak to someone who can advise me and knows about my case or get a call back by someone who knows my case within 48 hours like the phone line says. I would like contact from medical professionals checking on my well-being especially when I have started/changed my medication. I don't want to be passed off and discharged from the service without any exploration to whether my mental health conditions have been properly diagnosed and I have a treatment plan on place. I have never had a full psychiatric diagnostic assessment and this has so far resulted in years of misdiagnosis and mistreatment.
