I told Nurse that my patient who I have had privately for 4 years was declining due to terminal cancer. I said he has around 3 weeks left but was still not admitted into hospice. I only involved them due to me No longer doing him as a 1-1. Next day he was admitted into QEH, went to 3 different departments and then last days at ward 25 (discharge lounge) where he received poor care and attention. I stayed 1 night and still visited every day and still cleaned him every day due to him being wet or had opened his bowels. My patient did not have 1 sore on him until he was admitted and got a pressure ulcer. He received excellent care. Why at this point was he not taking to a hospice. I was told because he was drinking still?? Every human is different and he loved his tea He was 92 years old and I promoted independence until a week before he passed and he could not manage.

Thank you for your feedback. I am responding as our Director of Care and Service Transformation is currently on holiday. As the patient has died, I have discussed the case and your feedback with the patient’s son. I understand he has had a conversation with you and will feedback to you following our conversation. I can also see that much of your feedback relates to the care received during a stay in hospital, so I have contacted the Assistant Director of Nursing to provide feedback and some suggestions for improvement. I have asked our Director of Care and Service Transformation to ask the team to reflect on whether anything might have been done differently to avoid a hospital admission, so that we improve care for others going forwards.
