Caring staff given time to get to know patients. Team work across disciplines to give total care. Earlier warning of possible discharge.

thank you for your lovely feedback, in regards to notice of discharge, it is sometimes difficult to give lots of notice, depending on the service continuing care in the community, we endeavour to give as much notice as possible.
