Timing of visits was not always totally helpful for me. Emotional support good.
Thank you for your feedback. The Hospice at home team do everything they can to accommodate the timings that patients prefer but this is not always possible. When staff from the Hospice at home team visit, they have a full understanding of what might be needed, but that first visit may identify the patient requires a care package if one has not already been arranged. The Hospice at home team will then process paperwork to request for a care package to be put in place. Occasionally Hospice at home are requested to support patients where there is a gap between the patient being discharged from hospital and a care agency being available to provide the care. The Hospice at home team are not the ones who provide the care package, they assist in applying for the care package
