Suggested improvements
It would have been nice if you or all the other agencies involved would have reported his palliative care so the finding was not left to me.

Thank you for your feedback, unfortunately it is not possible to comment on how other services or agencies report palliative care needs. As a service we continue to work alongside many other services and continue to explain how our service works and when they should be referring to our service.
