It took the worry from me and my family about where to go and what to do after my long stay in hospital, you put us in touch with all the relevant people who could help us instead of the family wondering about who to go to for support. It was so good to have the help in changing my bedroom and providing the relevant furniture. I appreciate the regular visits to help me and check that all is well. I hope it can be adopted nation wide after hearing my another's experience of the NHS in Hampshire. Thank you.
