The support provided during the last couple of weeks of my mothers life was invaluable. My mother has faith that the team would do the best for her and they did, as a family we felt supported too.

I am sending this feedback for my husband who has since passed away.
All the time he was attended by the palliative care team he had the very best care he could have had. Everytime the came to our home they actually spoke to him and involved him in everything. I think everyone who came to see him and all people on the phone were amazing.

We feel supported by people who understand and care & seem to listen and get on our specific concerns rather than treat my mum as one of a number.
