You failed to inform me that that my daughter had not attended sessions (she told us that you had not contacted her)to help her with her mental health. The first I heard about her non-attendance was when we received a demand from a 3rd party debt collector for the sessions that she did not attend.
You told me that you were unable to update me about my daughter's health.
You sent me a confidential report about another patient - I pointed this out to you, however did not receive any response or acknowledgement.
