I have recently seen two GPs for two different issues. I felt extremely happy with the outcome of the appointments and felt hopeful about receiving support. Since then, both GPs have failed to follow up as promised and outcomed appointments/test results as no further action. One GP has documented incorrect information and support which I specifically did not agree to within the appointment.

Suggested improvements
Your GPs need to complete the actions as agreed with their patients in the appointments. Why involve a patient in a plan and mutually agree actions and not do this? Information documented (which can be seen by patients on the NHS app!!) should accurately reflect what was discussed in the appointment.
