I have got appointments where the Gp has not called Telephone appointments have a window of 3 hours. You might be asked to come into the surgery 20 minutes after the call. So you don't know where you stand all day Receptionist are expected to make decisions, whether you deserve an appointment and are expected to read you notes and tell you what to do.

Suggested improvements
Shorter windows for freephone appointment Make appointments for next day if expected to come in. Receptionist should not be making decisions about patients
