I requested my records from the Trust when I discovered this doctor made a diagnosis without even meeting me but based on some brief history (incorrect) shared. Such incidents are a major concern as these records or diagnosis are left for years to some other clinician to eventually reference the content.
More importantly, I was not even aware of such a diagnosis which breaches all data protection and governance rule.
Any consultant who makes a diagnosis this way should be struck off.
