things done not organised with the GP and crisis team. An example medication ran out, which I needed, I was three days out. I came to the hospital and collected the prescription in person. GP could prescribe the medication.

Suggested improvements
Organisation and the way you communicate with the patient. Unknown numbers, who do you phone back. Emails could be used instead of phoning me.
