Dear Sirs I would like to write to the board responsible for procurement of diabetes medicines at Barts Health NHS Trust. In particular, I wish to write to the trust with regard to funding for Abbott Freestyle Libre sensors for Type 1 Diabetics and give evidence why, with the scheme having been approved by the NHS ‘Drug Tariff’ on 1/11/17, the sensors should be funded for Type 1 diabetics in the Barts Trust constituency. I am a 31 year old type 1 diabetic and have used blood testing strips to control my condition for 17 years before changing to the Freestyle Libre. I have used and self funded treatment with the Freestyle scheme now for almost a year and it has dramatically improved my control. Please confirm who I should best write to and their address if I wish to give evidence and make submissions pursuant to having the Freestyle system prescribed by doctors in Barts. My name is Jack Swadling, my mobile is 07717665467 and email address is jack.swadling@gmail.com. I look forward to hearing from you. Kind regards Jack
