Palliative care were involved from the very beginning of discharge home.
Terri was excellent support and treated my mother with dignity and respect.
As a trained nurse myself it was difficult to 'handover'care to others but Terri listened to my concerns and included me in all aspects of my mothers care and dignified, pain free death.
Thank you to all of you.
