I have undertaken a review of care received primarily from 2012 - 2013 but in fact is ongoing. The Practice Lead and the Staff have been instrumental and very clear in enabling me to have access to high quality care as a patient, but also as a reviewer. This has been to provide access to copies of all of my GP medical records and in-depth discussions based on my capacity for decision-making during a period of Mental trauma. I have been granted access to speak to my GP and together to rifle through medical records and discuss my history. Due to excellence in upholding transparency and protection of information, I am privileged to be able to make these assertion. -Appointments have been easy to make and I have been warmly received. -Care has been consistent and across many different changes in medication, my opinion has been valued and well followed through. The records have been succinct and in depth - the only critique I hold is that repeated prescriptions do not refer directly to the original diagnosis as succinctly and clearly as I would hope. I can only offer that a more strict referencing system may be used when different Dr's issue repeat prescriptions so that during long-term periods of medication, the original diagnosis is observed or coded. All in all, this is a very small critique over a period of excellent care and an open door, with welcoming and engaging staff, that has been most appreciated.
