Terrible communication, especially letters, inviting you to take up services that don't exist or are not available in the time frame described. The reception staff assume you 'know the ropes' about how the health system works and can make you feel foolish by their tone of voice if you're new to it all. They should also speak more quietly if they are going to talk about people, semi-hushed tones didn't/don't quite do it.
