The quality of the service has deteriorated over the last few years. Standards have definitely slipped. Even the appearance of the reception areas is tired. Mistakes are being made regularly. Staff seem to be too under pressure to give good quality care. My original doctor retired, I have not met my new doctor. In the last 18 months I have had some significant medical issues, but I have only had one face to face appointment and that was with a locum, all my other appointments have been over the phone. I am aware that, due to my medical issues, I should have been offered other support and appointments, none of this has ever been discussed with me.

Suggested improvements
My guess is that you either have a staffing issue or a budget issue, or both. I have absolutely no idea what my doctor looks like as I've never actually met them, but I am not happy with the mistakes that have been made, or the lack of support. It comes across that my doctor has limited experience and is struggling with the workload, you are failing the doctor and, therefore, failing your patients. Get the practice back to where it was a few years ago before it becomes too far gone.
