Very clear and concise in conveying points. Double checking patient agrees is good - as well as checking if carer agrees too. Clear demonstrations of what is going on, with comparisons to previous appointments also clearly explained and linked.
Overall, very good and clear, perhaps dictation of the letter could be cut out of appointment, only as the same information is then run over another time, though this is only a minor possible adjustment.
