I have encountered Mr.Shaerf twice, and am very impressed. I have twice sustained comminuted intra-articular fractures of my right wrist - in 2 completely separate accidents, 6 years apart. I had long-term problems following the first injury, and more after the second. My first meeting with Mr.Shaerf was on the morning of May 9th, when I attended for 2 surgical procedures. Mr.Shaerf introduced himself as a member of Ms.Bremner-Smith's surgical team, and informed me of what was proposed. Though brisk, Mr.Shaerf's manner was not hurried, and after each step asked me if I understood what was proposed, and did I have any questions. All of this was very clear. Mr.Shaerf then informed me of the objectives and realistic expectations, and was very clear about specific limitations, and adverse consequences that sometimes occur - including Complex Regional Pain Syndrome, which he explained clearly. At every stage, Mr.Shaerf asked did I consent, and did not rush me at all for answers. I found this particularly reassuring as the department was extremely busy, and as a person who tries to avoid making fuss, I was very aware that Mr.Shaerf had many more patients to see. Mr.Shaerf addressed all of my questions and gave clear, concise responses with which I was totally satisfied. Mr.Shaerf informed me of the importance of keeping my fingers moving from as early as possible following the surgery, which I subsequently remembered and carried out. I was treated with respect and courtesy throughout. My second encounter with Mr.Shaerf was at a follow-up appointment in the hand clinic. Again, his manner was brisk but unhurried, and he invited me to speak first. This was particularly refreshing as it was an immediate indication of modesty and patience on Mr.Shaerf's part. I think that this is particularly valuable, especially when surgery is not a total solution. Previously (though not at the Chelsea and Westminster), I have felt intimidated by a surgeon, and was left with a sense of personal failure - despite understanding fully that my injury had been serious and that the gains of the surgery would be limited. On that occasion I had not been in any way disappointed with the outcome. Mr.Shaerf's approach demonstrated how different things can be. Again, Mr.Shaerf really listened to what I reported and addressed my concerns fully. This included further examination of my wrist, and setting out prospective courses of action, one of which was to continue with hand therapy only. I was allowed plenty of time to consider all proposed courses of action, and to ask questions - which were addressed and answered clearly and honestly. I enquired about the possibility of a further scan, as (subjectively) my wrist had deteriorated significantly over the previous 18 months - 2 years, and Mr.Shaerf explained that although that was certainly something to consider, it would not be beneficial at this stage, as the wrist needed more time to settle. Waiting would give a much better chance of identifying any ongoing problems, as distinct from any residual inflammation. I was completely satisfied with the clarity of this response. Mr.Shaerf demonstrated a very good grasp of the details and complexities of the state of my wrist. Although I am not a medical professional, I do have some understanding, and I endeavour to keep myself informed. It struck me that Mr.Shaerf was very well prepared for the consultation, and able to very quickly navigate the long record - including the extensive CT/MRI images and associated data. He seemed to be very quick at finding what was relevant for each query I made. Mr.Shaerf also enquired about and inspected the scarring, which I understand is an important detail in the type of surgery performed on my wrist. I was happy to report that staff at the hand therapy department were vigilant and conscientious with this. The appointment ended with Mr.Shaerf asking what was my preferred course of action, and allowed me plenty of time to decide, and enquire further. Informed by previous experience, I opted for cortisone injections and the possibility of further surgery if necessary. Mr.Shaerf ordered ultrasound-guided injections into 3 particular locations in the wrist, and a follow-up appointment at the hand clinic. When I went for the injections appointment, I read Mr.Shaerf's order, which was precise and unequivocal. It was clear throughout that my thoughts were to be given serious consideration, and I had every confidence that Mr.Shaerf was absolutely focussed on my best interests. Going on this experience, I would absolutely recommend Mr.Shaerf - he reflects very well on Ms.Bremner-Smith and others involved in his training. ................................................................................................................................................
