Hello Doctor, I would politely suggest that the form one filled in prior to the appointment could be improved to make it more relevant to male patients and hopefully more useful to yourself.

Thank you for suggesting this - I agree the form should be separate to the one we give to female patients - I am a new consultant at the unit and am working on improving patient information and forms currently. It is really useful to have your feedback to support this and it highlights that this form is insensitive to the fact that not all patients seen in the breast clinic are female.
