I made a referral to your service for a man in my care for SLT & Dietary advice. Triage contacted me quickly - listened to my concerns - assessed the man in my care and assured me they would get him help asap. They gave me advice also. I was contacted quickly by SLT team who advised to keep this man safe.

Completed referral by going to GP to get help - they made referral. Lorna called to explain who she was & how triage could help. discussed concerns - she took all details & completed a falls assessment she said. Lorna stated that she would inform Physiotherapy of our request for help and that there would be a meeting - we would get a letter after that.

We have a new lady living with us that we require OT support with. I made a referral and triage team contacted me that day. Listened to my concerns - gave me ideas and stated what would now happen with referral. Lady will be discussed at next meeting & we would get a letter.
