Because each of your questions relate to the medical staff each one of whom varied between very good to outstanding.

Suggested improvements
Your questionnaire is incomplete. It is restricted to the behaviour and performance of the medical staff and there are no questions concerning the organisation as a whole. In particular, you fail to ask how the administration and bureaucracy impacted on my overall experience of the NHS. In these areas there are some serious shortcomings. The organisation as a whole seems to be slow and cumbersome with a number of computer system inadequacies that make it impossible for the NHS to carry out some tasks that are possible from the average desktop or laptop computer. Medical staff's jobs are made even harder as they work to deal with the problems that arise from inadequate administration whilst caring for their patients. The NHS would benefit from a radical reorganisation of its management, part of which would be to replace civil servants with people from successful industries who are results rather than task orientated. If the NHS were to achieve the same level of efficiency as, for instance, Amazon, it would be less top heavy and more effective as efficiency improvements would release resources, presently being wasted in unnecessary bureaucracy, for front line medical care, where they are really needed.
