Review of Heart Failure Project

I was seen promptly after returning home from hospital and suffering from bouts of distressing anxiety and breathlessness. A doctor was arranged, meds prescribed and district nurse contacted all on the same day. This was followed up with phone calls to my daughter to check on how I was doing. All done with care, understanding and reassurance.
