They need to address the following failures and eview therapy services, in particular OT and their discharge planning. Poor communication between teams on the ward and discharge team and family No discharge planning Ignored and disregarded information from family Failure to communicate with patients wife on changed discharge date plans Admitted for rehab but discharged more immobile deconditioned institutionalised and dependent on others for all self care. 3 x hospital acquired infections No home visit by OT to assess equipment needs for discharge leading to return to hospital for patient. OT went with patient on day of discharge. As patient unable to get legs on bed said he had to return to ward. This problem was 1 reason for admission, had frequently been reported to them by family. A meeting had been held on ward with family and staff to discuss it. They requested height of the bed which family provided. However failed to identify this as a goal for rehab or work towards it. Then unbelievably they were surprised at discharge home he couldn't do it and had to be readmitted. Patient when eventually discharged needed full assistance mobilising and was still having falls. He was weak and required maximum assistance of 2 to walk and for self care. 2 weeks after discharge with family input and reablement physiotherapy and equipment provided after urgent OT assessment 2 days after discharge he is now independently mobile with frame, self caring and Is able to get on and off bed independently.

I'm Alan h I was discharged about 3 days ago to be onist it's great to home even though iv been having iot of times since back home but that's expected .y mental health problems but it was the. EST thing I have ever done being in Clifton please send my streangh and happiness to all the nurses doctors physios volanteas etc feel really bad fr all of you working in this heat and those .asks your all amazing beautiful incredible Angles you r all the true and heroes and princess please remember you r all priceless can someone please tell staff nurse Susana or earin ward 1 I'm in the process of drawing a picture for the hospital and a special picture fr evalyn and Lucy also got a lot of past drawing please let me know somehow of URL except these again thank you for helping me through all of this u deserve something that money can't buy so I send my love thanks respect and onisty lov Alan

I would love to send a very special thank you to all on ward1 and everyone else not on ward1 what with my mental health problems etc please forgive me for being such a bad patient u all go through the worse things I can't even imagine ur patients and advice and care and comfort all the nurses and volantease and doctors and physio gave me especially with my mam passing away while I was there I can't thank you enough please pass my love streangh and happiness to all on and off the ward I will be coming back to bring few drawing s fr Lucy Evelyn and one of the hospital etc please if poss send a msg to Evelyn fr all the help and advice she gave me regarding my drug and mental problems thank you Evelyn iv come dead close to contacting them again but I remember the promise we made and so I v thought twice thank you and finally I wish to send Michael all my love and thanks for everything u made my day so much happier and Al ur help and advice I hope everyone enjoyed the goodies my brother bought on my discharge see u soon alialan sooty /sweepxxx

I Would really like like to thank the doctor a nurse from the Dermatology dept for there care and attention they both gave me during some surgery I received today 9th Feb sorry I didn't get there names but made me feel really safe and they were attentive in these difficult times would you please pass on my kind regards onto them an excellent team who need to be recognised Regards. at 58yrs old i was told i was an old man and was refused a simple health supplement that would cure my osteopenia . due to other medical conditions a broken bone or fracture can kill me, so it is very important i have good bone health

I found the Nurses were lovely, although they appeared far too subservient to the 2 Dr`s. They ran around them unnecessarily like underlings, as if they were in fear of them. They were not allowed to make independent decisions and quite frankly I would have trusted them over the 2 Dr`s. I found it too hierarchical.

My mum was in a terrible state with her leg when she first came to the ward and just after a couple of visits and treatment her condition improved greatly and so had her demeanour. Her spirits had lifted and foot/ankle allergy soon started to show signs of getting better. The staff were and are amazing, they are kind, they listen and show empathy and offer great words of wisdom and advice. Special thanks and mention to Sara and Grant. The care and attention given is exceptional. Thank you on behalf of my mum.

For me the local nature of the facility is a massive bonus. The care is excellent and feels very personal. It is a very relaxed atmosphere which I am sure contributes to the success of the treatment! Staff and friendly and extremely knowlageble.
