possibly too much paperwork sent to me especially the last letter advising me of the appointment that I cancelled and the new date - only received two days after the new appointment. Suggest that the AAA Screening questionnaire could be handed to the patient at the end of the appointment and ask if they could fill in before leaving - this would then save cost of reply paid envelope. Suggest patients are advised Box 2 is optional and that it is just a fews ticks needed
