This is a really lovely place in the community. When 15 years ago I was upset about the way I was handled there was good follow up but I think now the communication and ethos is always amazing.
Suggested improvements
Put non binary not just male and female on this form.
But as far as the practice itself is concerned congratulations on getting good architecture as an indicator for your good care.
