Our son was under the care of Dr. J.Tanner. He was re-admitted to the Welland Unit on the 23rd May 2017 after a serious incident. He was a highly vulnerable patient and suicidal. He should have been closely monitored and given his history and his behaviour at the Unit which Dr. Tanner was fully aware of, he should not have been allowed out unescorted. He failed to return at his given time and the Missing Persons Policy was not implemented and therefore nothing was done to find out his whereabouts for hours. Later that day we received a visit from the British Transport Police to say that our son had taken his own life. I telephoned the Ward and they were completely unaware of what had happened. This was utterly appalling, disgraceful and unforgivable. Dr. Tanner did not listen to us as a family or our deep concerns for our son. We believed that he would be safe and looked after. The public need to be aware of the incidents that occur at this centre with patients going AWOL and suicides. As a family we are deeply traumatised and the pain and suffering at losing our son is going to be with us for the rest of our lives.
