This GP incorrectly completed an immunisation history form for occupational health, stating, 'Nothing recorded in the notes Checked old records - nothing recorded'. I went through my copy of my medical records and found documented evidence of some of them. Occupational health also found that I had immunity to Rubella, which supports that I had been vaccinated, as recorded in the copy of the medical records that I have. This GP obviously doesn't take her job sufficiently seriously to actually look for the vaccination information within the GP medical records, to ensure that she completes such forms accurately. I would not trust her.
